Advanced Oral Care Membership
How Does the Membership Work? 
For a flat annual fee, you will receive:
- Two regular dental cleanings or two perio maintenance cleanings.
- One emergency examination, periodontal evaluation
- Oral cancer screening
- Intra-oral camera examination
- ANY X-rays needed in the office
- 15% off additional dental cleanings
- 15% off most general dentistry procedures
- Fees are based on the usual customary fees at the time of service

What Are the Exclusions?
- Our membership is not a dental insurance plan, and can only be used at Dentsitry of Fishers.
- Membership is non-transferable.
- Membership cannot be combined with any other dental insurance coverage.
- Discounts under your membership do not apply to any treatment prior to joining.
- Some procedures are not eligible for the 15% discount. These procedures include bleaching, Invisalign, and Orthodontics.
