How Does the Membership Work? 

For a flat annual fee, you will receive:

• Two regular dental cleanings or two perio maintenance cleanings. 
• One emergency examination, periodontal evaluation
• Oral cancer screening
• Intra-oral camera examination
• ANY X-rays needed in the office
• 15% off additional dental cleanings
• 15% off most general dentistry procedures
• Fees are based on the usual customary fees at the time of service

• NO Claims to File or Wait On
• NO Limitations
• NO Pre-Authorization Requirements
• NO Pre-Existing Condition Limitations
• NO Denial of Coverage
• NO Waiting Period (immediate eligibility) 

What Are the Exclusions?

• Our membership is not a dental insurance plan, and can only be used at Dentsitry of Fishers.
• Membership is non-transferable.
• Membership cannot be combined with any other dental insurance coverage. 
• Discounts under your membership do not apply to any treatment prior to joining. 
• Some procedures are not eligible for the 15% discount. These procedures include bleaching, Invisalign, and Orthodontics.
